What's the code for a hurt passenger when a snowmobile hits a tree? What's the code for an infected 3rd degree burn on 8 percent of the right arm caused by scalding water (actually 3 codes - one for the infection, one for the burn and one for the scalding water)? What's the code for healthy baby delivered by cesarean at 32 weeks gestation when the mother has fibrous cysts (again, 3 codes)? What the code for a driver who's hurt when he's in a car that gets hit by a train (2 codes)? What's the code for a family history of diabetes? What's the code for a couple who comes in for artifical insemination, then decides they don't want to do it? And the codes are pulled out of a book that's about 2 inches thick and it's updated yearly. Apparently there are computer disks with the same info and I swear this is what the nurse teaching us said "Those disks are $300 and the book is $125 so we use the book.". OMG. Do they not realize they can save $175 in time the first freaking week??!! Even my Dr, who runs a pretty big practice in Alpharetta is just now computerizing all this. I have discovered that pretty much the vast majority of the medical world is governed by Medicare. Now I know why my Dr. doesn't accept it. |

Here are some I got wrong on the most recent test:
Diabetic retinopathy, type II, controlled. My answer: 250.5, 362.03 Correct answer: 250.50, 362.01 Palpitations due to overdose of monoamine oxidase. My answer: 785.1, 969.0 Correct answer: 969.0, 785.1, E854.0 So the nurse tells us "Actually, the insurance companies would have been fine with those answers.". Yeah, but the insurance company isn't grading me

This should be simplified and readily available in medical software with drop downs and check boxes same way auto insurance adjusts a wrecked car. I'll admit I couldn't do that one. Every bone in my body would be screaming out "this is stupid, re-engineer it" while I plodded thru it. Last time I had the occasion to visit a hospital and watch it work and all the processes, I had the same urges. They have people stacked up in the hallway in the ER and are diverting ambulances with people very sick if not dying all because the system is broken and inefficient. I'm not talking the computers specifically but the entire system of how a patient is moved thru from check in, to diagnosis, to testing to identifying available beds. It's all just broken. Part of that is government over complicating stuff with regulations, part of it is insurance companies not wanting to pay for stuff. But it's mostly lack of someone who understands basic system analysis being a position of power to look at the whole thing from the entrance to patient release ane making it flow smoothly. Companies that have to try to make a profit do this kind of thing or die. In healthcare, they do not do it, so the customer dies unfortunately. |
